Join us after the Easter Break for a Post 19 Transitions Coffee Morning. Alice Boddy, Post‑19 Co‑Production Coordinator, will be joined by the Transitions Team for Hammersmith and Fulham for a relaxed coffee morning for parent carers of young people transitioning to adulthood. The session is appropriate for any parent-carer with an EHCP aged 14+.
We’ll support you to understand the next steps for your young person, including Preparation for Adulthood. Come along to ask questions and find out more about the support available in Hammersmith & Fulham as your young person moves into the next stage.
